I just found a very useful add-on yesterday.  It's called Librameister and will equip the correct libram (as long as you are carrying it) depending upon the spell you are casting.  I have the Blessed Book of Nagrand (Little Heal) and the Libram of the Lightbringer (Big Heal), and it worked great in the BGs  I ran last night.  I could hear them being equipped.

The reason you can equip after you've started casting is because the bonus doesn't get added until the cast is completed.
